Patient Care​

​

Residents will spend 70% of time in direct patient care related activities from: MTM, outreach events, immunizations, medication assistance programs, CDMT, DSMES individual and group opportunities, and TCM. Residents will also engage in patient centered dispensing activities at a minimum of 8-12 hours/week.

Leadership & Management â€‹

​

Residents will be engaged in several leadership opportunities including: 

  • Pharmacy work groups/committees (i.e. Chronic pain, COW (Clinical Operations Workgroup), Outreach, Diabetes DSMES Group)

  • Community Service Requirement - DSMES/Outreach

Professional Involvement​

 

Residents will attend scheduled organizational meetings and have professional involvement including, join and actively participate in ASHP, along with recruitment activities for the residency as well as join and participate in other local, state, or national pharmacy organizations.

Community Based Practice & Improving Patient Care​

 

Residents will have the opportunity to create and implement a new collaborative practice agreement (CPA) or enhance a current CPA as well as complete a research project and business plan. 

Teaching & Education​

 

Residents will have the opportunity to earn a teaching certificate as well as precept APPE students with guidance from the RPD. Residents will also gain independence in teaching to medical residents on chronic disease state management such as diabetes, COPD and heart failure.

Community Service

 

Residents will have the opportunity to work with our outreach department to service our community in a multitude of ways, including vaccination clinics, healthcare on wheels, and community education.

Condensed/Block Learning Experiences​

  • Orientation: 4 weeks, 5 days, (40-50 hours per week)

  • 340B Pharmacy/Health Center Administration: 6 weeks, 1/2 day (4 hours) per week

Longitudinal Learning Experiences

​​

  • Patient-Centered Dispensing: 12 months, 1 day (8-12 hours) per week

  • Transitions of Care Management: 12 months, 3/4 day (6 hours) per week

  • Chronic Disease State Management: 12 months, 2 days (12 hours) per week

  • MTM: 12 months, 1/2 day (4 hours per week)

  • Advanced Community Precepting: 12 months, 1/2 day (4 hours) per week

  • Project Management: 12 months, 3/4 day (6 hours) per week
